Health Navigation Program
Indigenous program
The Health Navigator provides support and advocacy for First Nations, Inuit, Métis and Urban Indigenous community members by:
- Facilitating and coordinating access to health care services
- Addressing cultural and spiritual needs – Ability to refer to services of an Elder
- Networking with Indigenous and non-Indigenous partners to make the health journey a culturally safe experience
- Assisting in filling out forms and acquiring necessary documents (health card, hospital card, Indian status card)
- Accompanying to appointments
- Advocating for patients needs
- Making sense of the system
- Referring and connecting to other services
- Translating (from English to French) during appointments
