Location: Montreal (on-site position with travel to hospitals across the island)
Status: Full-time temporary position until March 31, 2026
Schedule: 35 hours per week, Monday to Friday, 5PM to 12AM
Salary: $28.98/hour
Position Summary
The Community Health Navigator supports First Nations, Inuit, and Métis individuals experiencing or at risk of homelessness in accessing culturally safe physical and mental health services. The person acts as a bridge between Indigenous patients and healthcare institutions, provides cultural and emotional support, ensures medical follow-ups, and collaborates with community and hospital partners.
Key Responsibilities
- Accompany Indigenous patients in Montreal hospitals
- Coordinate appointments, admissions, and follow-ups
- Provide cultural interpretation and advocate for patients’ rights
- Refer individuals to appropriate services and resources
- Develop individualized action plans
- Maintain records and communicate with the team
Qualifications and Skills
- Minimum of 2 years’ experience in intervention, social work, or a related field
- Strong understanding of Indigenous realities and intergenerational trauma
- Crisis management and support skills
- English required; French is the working language; an Indigenous language is an asset
- Experience in community settings or working with individuals experiencing homelessness is highly valued
