Job Title: Weekday Community Health Navigator
Employee Type: Full-Time Permanent
Reports To: Community Health Coordinator and Program Manager

 

Job Purpose

As a Community Health Navigator at PAQ, you will play a vital role in supporting Indigenous individuals experiencing homelessness or at risk of homelessness in accessing culturally safe healthcare services. Working closely with community members, hospitals, and healthcare providers, you will provide advocacy, emotional support, and guidance throughout the healthcare journey. This role is a blend of outreach, intervention, and patient accompaniment, ensuring that Indigenous patients receive equitable and respectful care.

As part of the PAQ team, you will receive specialized training in intervention strategies, suicide prevention, trauma-informed care, First Aid, and Indigenous cultural knowledge. Your work will directly contribute to improving health outcomes and promoting the well-being of Indigenous community members in Montreal.
